What a Doctor Does, and What a Dietitian Does
It is worth separating the two jobs, because people often book the wrong one and lose a few months doing it. A doctor examines you, orders tests, treats disease and prescribes medicine. A dietitian takes your diagnosis, your reports and your routine, and turns all of that into what you actually eat on a Tuesday. If your thyroid is underactive, only a doctor can start you on medication for it. Once you are on it, someone still has to build the eating plan, and that does not happen in a ten minute consultation.
Plenty of people need both, and there is no conflict in that. Ayesha Nasir works alongside whatever your own doctor has already started, not around it.
When You Should See a Doctor First
There are situations where a diet plan should not be the first step. If the weight has climbed quickly without any change in your eating, if you are always cold and tired and losing hair, if periods have stopped or turned irregular, if there is diabetes in the family and your sugar has never been checked, or if weight is dropping without you trying, get a doctor to look at you before anything else. The same applies if you are on steroids, antidepressants or any hormonal treatment, because some of those move weight on their own.

If Your Doctor Has Already Put You on Medication
A fair number of clients now arrive already on a prescription, either for weight or for diabetes. The plan is built differently for them. Appetite is often very low in the first few weeks, so the priority becomes getting enough protein and fluid into a much smaller volume of food, keeping fibre up so digestion does not stall, and protecting muscle so that what you lose is mostly fat. Anything to do with the medicine itself, the dose, the side effects, whether to carry on, all of that stays with your doctor.
What to Bring to the First Consultation
- Reports: whatever you already have. A thyroid profile, fasting sugar or HbA1c, lipid profile, vitamin D and a CBC cover most cases. Please do not go and get tests done just for this appointment. Bring what exists.
- Medicines and supplements: the names of anything you are taking, including anything suggested by a hakeem or a gym trainer.
- Your history: the diets you have tried, what worked for a while, and what you gave up on and why.
- Numbers: your current weight, and your highest and lowest adult weight if you can remember them.
- Your routine: work hours, sleep timings, who cooks at home, and how often you eat out.
