I want to be clear about one thing first. I do not sit in any hospital or clinic in Multan, and I am not attached to a practice here. My only consulting room is Holistic Care in Sector Y, DHA Phase 3, Lahore. Multan clients work with me over video, which is honestly the sensible option when the alternative is a five hour drive each way. Mango Season

Nobody who grew up around Multan is going to eat two slices of mango in June and feel satisfied. Mangoes arrive by the crate, they are better here than anywhere else in the country, and for eight or ten weeks they replace whole meals or get added on top of them. A mango shake made with full cream milk and sugar, after dinner, most nights, is enough on its own to stall a plan.

I do not cut mangoes out of a Multan diet plan in mango season, because that advice gets ignored and then the whole plan gets ignored with it. We fix the amount, fix the time of day, and take the calories out of somewhere else in that meal. Eaten whole rather than blended, and put where they belong in the day, they cause much less trouble than people expect.

Desi Ghee, Full Cream Milk and Lassi

A lot of households here get milk from a known dairy or from family land, and the food is cooked in real desi ghee. There is nothing wrong with either of those things. The problem is the quantity, because nobody measures ghee with a spoon when it comes in a tin, and a glass of full cream lassi at breakfast is a meal in itself.

So we measure instead of removing. You keep the ghee, at an amount I will give you in spoons, and we decide what the daily milk and lassi actually add up to. This usually matters more than anything else on a Multan plan, because it is the part people genuinely do not see as food.

Sohan Halwa, Guests and a Plate That Keeps Getting Refilled

Sweets come into the house whether you buy them or not. Guests bring sohan halwa, you send it to other people, and there is almost always a box open somewhere. On top of that, saying no to food at someone else's house is close to an insult here, and refusing twice is worse than eating it.

The practical answer is not willpower. Keep the sweets out of the room you sit in, because most of that eating is proximity rather than hunger. When you are invited somewhere, eat before you go so you arrive able to take a small portion politely, and treat that day as a planned heavy day rather than a failure. We build one or two of these into the week from the start, since I already know they are coming.

If You Have Been Told to Control Your Sugar

A lot of the people who contact me from Multan have already been to a doctor and been told their sugar is borderline, or that they are on the edge of diabetes and should fix their diet. Usually nobody explains what fixing it means beyond stopping sweets, which is not enough on its own.

What changes things is the shape of the whole day. How much roti and rice you are eating at one sitting, what you drink with it, whether protein is on the plate at all, and how long the gaps between meals are. I work on that. Your medicines and your test schedule stay with your doctor, and if something needs their decision I will tell you to go back to them rather than guess.

  • Send in advance: recent blood work including sugar and HbA1c if you have them, your medicines and supplements, and your current weight.
  • Describe the real day: the milk, the lassi, the ghee and the sweets included, not the tidy version.
  • Tell me about the kitchen: who cooks, whether milk comes from a dairy or the market, and how often guests are over.
  • Afterwards: the written plan comes on WhatsApp, and we review it after you have run it for two weeks.
